OTTAWA - APPLICATION REQUIREMENTS

Students applying to Ottawa MUST have all of the following in order to be considered:

1) a complete application form (pink)
2) a signed student/parent contract form (yellow)
3) $175 deposit (no postdated or NSF cheques)
4) at least $60 in fundraising money (Escape Weekend tickets)
5) DUE DATE IS MONDAY, OCTOBER 25 - 3 PM

Without all of the above, students will not be considered
